Step-by-Step: How to Add Signatures to Google Forms

Follow this 3-step guide to connect your Google Form and activate digital signature collection.

Step 1: Add a text question in Google Forms

  1. Open your form in Google Forms.
  2. Click the (+) Add question button.
  3. Set the question type to Short answer (or Paragraph).
  4. Label the question clearly (e.g., "Authorized Representative Signature", "Signer Signature", or "Digital Signature").
  5. Set the field to Required if a signature is mandatory.
